Medical Skilled Nursing
Registered nurses provide skilled assessments, teaching and medication management, wound care, IV infusions to adults and geriatric patients in the home setting. The nurses involve the patient, family, and care giver in the treatment plan. They routinely evaluate the patient’s condition and report findings to the physician. The nurses coordinate care with other health care professionals to meet the patient’s rehabilitation needs in the home.
Nursing Assistants
All of our nursing assistants (Home Health Aides) are certified. They work under the direction of a registered nurse or therapist to provide personal care for the patients.
Wound Care
Provided by skilled nurses who are specially trained to care for patients with wounds and incisions.
Diabetes Education
Registered nurses provide assessment and teaching for newly diagnosed and long-term diabetic patients. Registered nurses assist in planning the teaching programs to help patients and care givers balance food, medication, and exercise.
Rehabilitation Services
Physical Therapy
Physical therapists work to restore patients to their optimal level of function utilizing exercise programs, education, ultra-sound and other training.
Occupational Therapy
The occupational therapists provide teaching and training for patients and care givers in techniques to promote independence in activities of day to day living, such as dressing, eating, holding a cup of coffee and other independence.
Speech Therapy
The speech therapists evaluate patients with hearing, speech and swallowing disorders and design individualized plans to assist patients in reaching their highest potential.
Medical Social Worker
The medical social worker provides skilled evaluation of the patient’s social and emotional needs that directly affect their illness and psycho-social status. They also provide counseling for long- range planning and decision making while making every effort to identify and utilize community resources.
